A clinician completing an assessment and a colleague reviewing the care plan need a shared starting point. Keep the questions, findings and proposed follow-up connected, with a deliberate review before changing planned care.
Choose the instrument for the person
Review the questions, program scope and any scoring rules before starting. The library separates platform instruments from assessments authored by your organisation.
Administrators manage platform availability. Draft, archived and unavailable instruments are excluded from the clinical library.
Record findings in context
Complete the structured questions and review the result. For instruments with scoring configured, the assessment shows score completion and interpretation; unanswered scored fields remain visible.
Use the instrument’s instructions and clinical judgement. A calculated score is one input to the care decision.
Review what belongs in the plan
Publish the relevant recommendations for care-plan review. The team accepts or declines care-plan content in the plan’s review inbox, rather than treating every finding as an automatic change to planned care.
Where a new draft plan is started from an assessment, review the proposed content before approval or activation.

An assessment produces a proposed care-plan action. The reviewing clinician checks it against the person’s goals, current plan and circumstances before accepting or declining it.
The assessment provides the starting evidence. The care-plan review is where the team decides what should become planned care.

Which instruments can our team use?
The library shows approved instruments available to your organisation, taking account of program scope, administrator settings and current version status. It separates platform-authored instruments from your provider’s assessments. Review the current catalogue and the domains your team needs during the demonstration.
Does every assessment produce a score?
No. Scoring depends on the instrument’s configuration. Where scoring is configured, staff can review the rules, score completion and interpretation. Structured assessments without numeric scoring are also supported.
Do all provider-authored templates have platform RN sign-off?
No. Platform instruments can carry an explicit RN sign-off status. Provider-authored templates follow their own publishing path and are labelled as provider-authored; that label does not mean they have platform RN sign-off. Your organisation needs an appropriate clinical governance process for the tools it publishes.
Does an assessment update the care plan automatically?
Care-plan content recommendations are reviewed in the care-plan inbox, where the team accepts or declines them. Starting a draft plan from an assessment does not make its proposed content approved or active care.
Are these licensed copies of branded clinical instruments?
The library described here contains platform-authored and provider-authored instruments. We do not present them as licensed copies of branded tools or claim independent clinical validation. Your clinical leads should assess suitability and any licensing requirements for the instruments your organisation intends to use.
