Each carries its own scoring, a provider can enable or disable individual instruments, and providers can author their own instruments as well, all under the same approval and RN sign-off governance.

Are scores calculated and banded automatically?

Yes. Each instrument produces a score, and that score auto-bands to a severity level so the clinician sees where the result sits on the scale, not just a raw number. The scoring and banding are part of the instrument itself, applied consistently each time it is used.

How are assessment instruments governed?

Through approval, RN sign-off and version control. Draft or provider-authored instruments stay unavailable for clinical use until an RN signs them off, and the library is version-controlled — so the tool in use is the one the organisation has approved, and an auditor can see who approved it and when.
