Is the advance care plan a separate document or part of the care plan?

It is part of the care plan, not a separate document store. The ACP is recorded against the resident and surfaced inside the clinical workflows where decisions are made — medication review, escalation, hospital transfer — rather than sitting in a folder a clinician has to remember to open.

How are advance care plans kept current?

Plans are versioned. When a resident's wishes or circumstances change, a new signed version supersedes the previous one, and the history is retained in the record. The current plan is always the one in force, so there is no ambiguity about which version applies.

Can the family see the advance care plan?

Where the family has Family Portal access, the advance care plan can be shared with them so everyone is aligned on the resident's wishes. Visibility follows the access the resident and provider have granted — it is not exposed by default.

How does this support Strengthened Standard 5?

Standard 5 (Clinical Care) under the Strengthened Aged Care Quality Standards expects providers to deliver care consistent with the person's needs, goals and preferences.
